MASLD: The Silent Epidemic Inside the Modern Liver
Why the medical world dropped the name "Fatty Liver" and why your metabolic health is the key to survival.
From "Fatty Liver" to MASLD: What Changed?
For years, we called it Non-Alcoholic Fatty Liver Disease (NAFLD). But the name was misleading. We have now transitioned to MASLD (Metabolic Dysfunction-Associated Steatotic Liver Disease) because it identifies the true culprit: Metabolic Dysfunction.
In Bangalore, I see this daily—patients who don't drink alcohol but have a liver full of fat. This is driven by insulin resistance, high blood sugar, and obesity. Your liver isn't just "fatty"; it's a mirror of your metabolic health.

A simple Fibroscan can tell us your liver's health. Consult **Dr. Srinivas Bojanapu** at **Kauvery** or **Dhaara Hospitals**.
The Investigative Report: Myths That Kill
Myth: "I am thin, so I can't have MASLD."
The Reality: "Lean MASLD" is a significant concern in the Indian population. Even if your BMI is normal, you may have "visceral fat" around your organs. In Bangalore, our high-carbohydrate diet (white rice, sugar) causes fat to accumulate in the liver even in thin individuals.
Myth: "Fruit juice is the best way to detox my liver."
The Reality: Fructose (fruit sugar) without the fiber is a direct toxin to the liver. It bypasses the gut and goes straight to the liver to be turned into fat. **Stop the juices; eat the whole fruit.**

The answer is a resounding YES, but only if caught before advanced Fibrosis sets in. I prescribe a three-pillar approach to my patients:
- Weight Loss (The 7% Rule): Losing just 7-10% of your body weight can resolve liver inflammation and even reverse early scarring.
- The Mediterranean-Indian Hybrid Diet: Replace white rice and refined flour with millets, lean protein, and healthy fats like nuts and moderate ghee.
- Exercise as Medicine: 150 minutes of brisk walking per week acts as a "fat-burner" for the liver cells.
Questions You Should Be Asking Your Doctor
Don't just look at your SGPT/SGOT levels. If you have Diabetes, Hypertension, or a large waist circumference, ask for:
- Fibroscan (Transient Elastography): To measure exactly how much fat and scarring is in your liver.
- Fasting Insulin: To check if your body is struggling with insulin resistance.
- Lipid Profile: Specifically looking at your Triglyceride levels.
Important Note on Alternative Medicine:
Many "liver tonics" sold in the market are unproven and can actually cause Drug-Induced Liver Injury (DILI). When your liver is already struggling with MASLD, adding unregulated supplements can push it into failure. Always consult a Hepatologist before taking any "natural" cures.
