The Fatty Liver Reversal Protocol

The Bangalore Scenario: In our Electronic City clinic, 6 out of 10 IT professionals we screen have Non-Alcoholic Fatty Liver Disease (NAFLD). It is driven by what we call the "Tech Lifestyle"—sedentary jobs, late dinners, and high-carb foods.

Why Worry? Grade 1 is harmless. But if ignored, it turns into NASH (Inflammation). Once inflammation starts, your liver begins to scar (Fibrosis). This is the silent path to Cirrhosis.

Dr. Srinivas's 3-Step Reversal Plan:

  • Dietary Shift: We don't just say "eat less." We reduce fructose (sugar) and introduce simple proteins. Coffee (black, no sugar) is actually good for the liver!
  • Pharmacotherapy: For some patients, diet isn't enough. We may use Vitamin E or Saroglitazar to help the liver shed fat.

Jaundice: The Warning Sign

Jaundice (yellow eyes/urine) is not a disease; it is a symptom. In adults, it is rarely "innocent." We categorize it into three buckets:

1. Obstructive Jaundice

A stone or tumor is blocking the bile duct. This causes intense itching and dark urine.
Action: Requires immediate ERCP or Surgery. Native medicine will NOT work here.

2. Medical Jaundice

Caused by Viruses (Hep A, B, C, E) or Alcohol.
Action: Requires medical management, antivirals, or steroids.

Living with Cirrhosis

The word "Cirrhosis" sounds scary. It simply means "scarring." If you cut your skin, it forms a scar. If the liver gets injured repeatedly (by alcohol or virus), it fills with scars.

Compensated vs. Decompensated

This is the most important concept to understand.

  • Compensated: You have scars, BUT the remaining liver is doing its job. You feel fine. Our goal is to keep you here for 10-20 years.
  • Decompensated: The liver can no longer cope. You develop Ascites (water in belly), blood vomiting, or confusion. This is when we start discussing Transplant.

The Transplant Decision

As a Transplant Surgeon, Dr. Srinivas's advice is honest: "A transplant is a marriage, not a date." It is a lifelong commitment to medication.

We only recommend it when:

  1. Your MELD Score (a calculation of blood tests) is > 15.
  2. You have had a life-threatening complication like a massive bleed or liver cancer.
  3. Your quality of life is severely poor.
